Add Ladder design working doc The real Ladder model turned out bigger than what lanes-and-ladders.md currently describes: a code-shaped tree (repo/directory/file/function/ line rungs), not a Lane-decomposition tree, with drill-down history pulling in PRs, issues, comments, and docs, plus cross-repo dependency reach. Tracking that here while it's actively built, like competition-plan.md tracks the build, rather than thrashing the polished docs during development.
