Scaffold a real zz CLI: ladder build and ladder dump Adds `zz ladder build <owner/repo>` (generic onboarding against any real GitHub repo, via the same engine/ingest.ts functions onboard-bulk.ts uses) and `zz ladder dump <owner/repo>` (materializes a Ladder as text, JSON, or a self-contained HTML page). ladder-build skips any rung that already has a note unless --force is passed, so running it generically never clobbers hand-enriched content like supertest's. Consolidates all real data-gathering (gh api fetches, tag resolution, dependency parsing, and a new mechanical walkFileTree) into named, reusable functions in engine/ingest.ts, and refactors onboard-bulk.ts to call them instead of duplicating the logic inline.
