Check per-ref push status, not just the aggregate ok flag isomorphic-git's push() returns an aggregate ok plus a per-ref status map; the docs say explicitly there "can be a mix of ok and error messages" across refs. pushNote only checked the aggregate, so a rejected ref update could still report success here. Didn't turn out to be the actual cause of new repos' Ladder notes going missing (confirmed live: per-ref status also reports ok:true for the failing case) -- that's a separate, deeper issue documented inline on readRung, not fixed in this pass. This check is still strictly more correct than before and worth keeping regardless.
