Build the Lane/ASK/ANSWER protocol engine; onboarding replays through it Adds engine/lane.ts (proposeLane/ask/answer/mergeLaneLadder) -- the Lane lifecycle that was always owed but never built, operating on the existing Lane/HistoryEntry types. Role-checking is deferred to a later step; this is step 1 of the planned local-process -> web API -> MCP server progression. Adds engine/model.ts: a local-only Ollama model backend (BYOK deferred). Nothing here ever shells out to another CLI -- that's what caused the earlier quota incident. Rewrites cli/commands/ladder-build.ts so onboarding actually drives the Lane engine: one local agent plays both sides of the ASK/ANSWER protocol per history item (claim, do the work, ask to merge, merge), instead of writing Ladder rungs directly. The repo itself is modeled as its own long-lived Lane that every onboarded item originates from. Reinstates three zero-cost data fetchers in engine/ingest.ts (fetchThreadComments, fetchContributorCommitMessages, findDependencyUsage) that were removed in an earlier revert only because they'd been bundled with unbounded LLM calls, not because they were wrong. Fixes two bugs found while verifying against jonschlinkert/is-number: a self-referencing Ladder note (the repo root was being appended as a child of itself, causing an infinite loop on read) and a model prompt that omitted an issue's actual author, causing it to invent one.
